This publication is designed to address the often unexpected challenges many seniors face when returning home from a hospital stay. Failure to manage medications, make follow-up doctor’s appointments, and obtain the physical assistance they need can often lead to seniors’ readmission to the hospital. Planning ahead of time before a planned or unexpected hospital visit can help patients address questions that arise during the discharge process and ensure a smooth transition home.
