Web Site | 410-837-2050 SAGECAP Baltimore is a unique partnership between SAGE and The LGBT Health Resource Center of Chase Brexton Health Care. The SAGECAP (Caring And Preparing) program offers caregiver support groups, one-on-one counseling, resources, referrals, assistance with financial and legal matters, and linkage to medical care for LGBT Older Adults and those who care for them. We also produce monthly Lunch & Learn events open to the community. Eligibility: SAGECAP supports informal caregivers who identify as LGBT themselves and/or care for someone who is LGBT. Some services are available directly to LGBT people over the age of 50. You do not need to be a Chase Brexton patient to take advantage of the SAGECAP program.
